Card A Player
Note to all coaches involved: Please keep cool in difficult situations. Be a good role model. Also, keep in mind refs have a difficult job.
Procedure to follow if a red or yellow card is issued.
Coaches from both teams are to report any player or players that receive a red or yellow card within 48 hours of the game using this form.
The commissioner will be responsible for notifying the club rep or president of clubs to let them know if the carded player is not permitted to participate in the next game.
Before you leave the field, acquire needed information
Player name and jersey number.
